Step 1.

Initial Call

You (or your loved one) can begin by giving us a call. During this conversation, we’ll gather some basic information to better understand your needs and determine if our program is the right fit. Occasionally, we may recommend that an individual receive a higher level of care or attend a detox program before admission to Evolution House.

To begin the process, we’ll need:

  • Your full name
  • Date of birth
  • Insurance information

Step 2.

Step Down

If You’re in a Treatment Program (Level 3.5 or Higher)

If you are currently enrolled in an intensive inpatient program (ASAM Level 3.5 or higher), your discharge planner can send your information directly to our admissions department to streamline the transition.

Step 3.

Screening Call

Once we’ve confirmed bed availability, we’ll schedule a screening call with a member of our clinical staff. During this call, we’ll ask questions about your history of use and recovery goals to ensure that Evolution House can best meet your needs.

Step 4.

Acceptance & Transportation

After the screening, if you (or your loved one) are accepted into our program, our team will help coordinate transportation to Evolution House. We’ve found that individuals who transfer directly from another facility (bed-to-bed) often experience the strongest start and greatest success in their recovery journey.
